Necrobiosis lipodica diabeticorum (NLD) is classified as a palisading granulomatous process. The areas of collagen alteration or “necrobiosis” can be found throughout the reticular dermis. Early on, the collagen shows fragmentation and clumping and later becomes more homogenous, hyalinized, and acellular. These zones of necrobiosis are surrounded by palisading histocytic cells and lymphocytes.
